About the role
The Facilitator is responsible for coordinating intensive, family-centered, community-based services for approximately 8-11 families in the Wraparound program. This position facilitates Child and Family Team (CFT) Meetings and guides the evolution of the Individual Child and Family Plan.
Responsibilities
- Effectively manage and coordinate all aspects of the Child and Family Team (CFT).
- Perform initial and ongoing strengths-based assessments of children and families.
- Identify and prioritize family’s needs across all domains.
- Schedule and facilitate all Child and Family Team (CFT) meetings; involve the family in the planning and decision-making process.
- Role model positive communication and conflict resolution skills.
- Identify opportunities and resources to promote family strengths, self-sufficiency, and independence.
- Carefully document adherence to all program and contract standards for compliance, productivity, and timeliness.
- Demonstrate leadership of the multidisciplinary Wraparound team and utilize a Team Concept to foster effective collaboration.
- Interface and collaborate with various child-serving agencies and community liaisons.
- Facilitate individual, group, and family therapy sessions as per the Individual Child and Family Plan.
- Provide psychotherapy to commercially insured clients through South Coast’s Hope Healing Connection Program.
Requirements
- Facilitator I: Bachelor's degree in Social Work, Marriage and Family Therapy, or Psychology; 2 years of experience in human service field (preferably Wraparound model).
- Facilitator II: Bachelor's degree in Social Work, Marriage and Family Therapy, Psychology, or related field; 2 years of experience in human service field (preferably Wraparound model); matriculating in a master's program.
- Facilitator III: Master’s degree in Social Work, Marriage and Family Therapy, Psychology, or related field; experience in human services (preferably Wraparound or similar program); registered with BBS.
- Facilitator IV: Master’s degree in Social Work, Marriage and Family Therapy, Psychology, or related field; experience in human services (preferably Wraparound or similar program); registered with BBS and obtaining clinical hours.
Knowledge, Skills & Abilities
- Familiarity with the functionality and phases of Wraparound programs.
- Proficiency in Microsoft Word, Excel, and navigating EHR systems.
- Knowledge of crisis management skills.
- Able to remain organized and manage multiple tasks at once.
- Able to establish and maintain appropriate rapport with children, youth, and families.
- Knowledge of behavior modification, child development, and/or the child welfare system.
- Ability to adopt a Team Concept and work effectively as a member of a multidisciplinary team.
- Experience with leading and facilitating groups and meetings.
- Knowledge of local community resources for families.
Physical Requirements
- Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances to ensure safe transport to client settings and to safely transport clients to appointments as needed.
- Senses of vision, hearing, and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
- Regularly required to sit, stand, climb stairs (1 or more flights), and walk while in route to and during family visits.
- Regularly required to sit and use computer in office settings.
Salary
$60,320.00 - $70,317.00 yearly
