Wrap Facilitator - CMHI/CMHW - Social Services

About the role

The Wrap Facilitator plays a crucial role in the comprehensive care of clients, focusing on strengths-based assessments, plan development, service implementation, and ongoing monitoring.

Responsibilities

• Completes comprehensive strengths-based assessments and collaborates with team members to develop a plan of care.
• Oversight of the implementation of the plan, including identification of service providers and family-based resources.
• Facilitation of monthly community team meetings and regular home visits.
• Maintenance of fiscal accountability by monitoring all services and authorizing care.
• Ensuring care delivery aligns with strength-based, family-centered, and culturally competent values.
• Consultation and education for providers on the model's values.
• Monitoring progress towards treatment goals and gathering necessary data for evaluation.

Qualifications

• Minimum Bachelor's degree in Social Work or related human service field.
• Two years of experience working with youth and/or families in social service or education settings.
• Experience in case management and team building.
• Knowledge of and respect for mental health issues and a commitment to empowering families.
• Strong communication and writing skills, especially in Spanish.
• Certified in CANS within 45 days of hire and maintained thereafter.
• Highly organized and detail-oriented.
• Valid driver's license and auto insurance.
• Ability to communicate effectively with various internal and external audiences.
• Proven ability to manage multiple priorities simultaneously.

Pay

Hourly Rate: $22.35 ($46,500 annually)
