• Built referral source relationships in various settings like hospitals, clinics, and senior-service organizations within the community.
• Worked to increase census and improve quality mix by engaging with acute care facilities and other referral sources.
• Provided prospective residents and/or their representatives with necessary information and support for choosing a nursing center.

Qualifications

• Graduate of an approved Licensed Practical/Vocational Nurse program and licensed in the state of practice.
• One-year experience providing specialized treatments in long-term care settings.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Current BLS/CPR certification (required for all SNFs; ALFs as required by state).

Responsibilities

• Conducted resident skin assessments and administered treatments as per treatment orders.
• Documented findings and communicated with physicians for new or revised treatment orders.
• Maintained records of pressure ulcers and skin alterations, including risk assessments, prevention, and monitoring activities.
• Performed weekly pressure ulcer assessments and measurements, and documented results.
• Identified and implemented preventive interventions for at-risk residents, including new admissions and individuals with skin alterations.
• Educated newly hired nursing staff on Skin Practice Guide requirements and provided access to the Skin Resource Library.
• Maintained supplies for wound care and coordinated their ordering with central supply.
• Aided the Director of Nursing and medical director in reviewing cases involving center-acquired pressure ulcers.
• Maintained awareness of regulatory changes and industry standards, and participated in ongoing specialized education.
