Job Summary
The ideal professional will be a clinically skilled and organized individual with experience in wound and skin care management, and a strong understanding of best practices in patient treatment and documentation.
EDUCATION, LICENSURE, AND EXPERIENCE
- Required: Graduate of an accredited nursing program
- Required: Current licensure in state of practice (RN, LPN)
- Required: Current wound care certification
- Required: Current CPR certification
- PREFERRED: Previous experience in wound care or skilled nursing setting
Essential Functions
- Perform comprehensive wound and skin assessments for assigned patients.
- Develop and implement individualized, evidence-based wound care plans.
- Collaborate with physicians, LIPs, and interdisciplinary team members to coordinate care.
- Monitor and communicate changes in patient condition to appropriate staff.
- Educate patients, families, and staff on wound care practices and prevention.
- Provide guidance and mentorship to nursing staff on wound care procedures.
- Maintain accurate and timely documentation in compliance with regulations.
- Participate in care planning, chart reviews, and QAPI activities.
- Ensure appropriate wound care supplies and equipment are available.
- Follow up on labs and treatment outcomes as needed.
