Wound Care Nurse

Responsibilities

• Work with the center clinical team and, under physician orders, develop and implement a wound care/treatment plan that meets each resident’s specific needs and reflects best practices.
• Accurate and comprehensive wound/skin care assessments of each assigned resident.
• Develop and implement an evidence-based care plan for each wound care patient.
• Communicate patient clinical status changes to the healthcare team and physician.
• Visit in-room with staff for collaboration, education/training, evaluating competency, and performing skills checks.
• Review charts of wound care patients as assigned.
• Deliver skilled nursing care.
• Manage nursing duties on a rotating basis.

Requirements

• Currently possess a nursing license in the state of employment.
• Possess active wound care certification.
• Maintain current knowledge of federal, state, and local regulations and inform on appropriate issues.
• Maintain current wound care/treatment practices.
