About the role
The Wound Care Nurse (LPN) - PACE at Jordan Valley Health is responsible for providing direct and indirect nursing care to assigned participants. This position works under the supervision of the Jordan Valley Senior Care Home Care Coordinator or designee but is independent in applying advanced nursing knowledge and skills.
Responsibilities
- Reviews participant’s overt problems/needs and assists in setting priorities.
- Identifies problems requiring further referral and/or follow-up; observes and records latest diagnostic results; performs advanced nursing observations using critical thinking skills.
- Affixes to developing and following a plan of care based on nursing process and incorporating the plans of other disciplines and continuing care needs; includes the participant/family in developing or revising the plan of care; and makes referrals to interdisciplinary support services.
- Completes initial and periodic assessments (minimally every 6 months) prior to scheduled team meetings.
- Communicates participant changes to center manager/team members.
- Ensures care provided conforms to accepted nursing practice standards and organizational/department standards; provides correct treatments/procedures and other care as prescribed and according to participant care standards.
- Demonstrates thorough understanding of age-related characteristics, symptoms, and needs of participants served; explains nursing/clinical procedures in appropriate way; provides participant and family education in coordination of 24-hour care delivery; identifies emergency situations and independently initiates advanced therapy; acts as primary coordinator of participant care with other health care personnel and evaluates participant care measures instituted.
- Works with primary care providers to furnish preventive, diagnosis, and therapeutic services prescribed for participants accepted for care by the clinic and implements physician orders as necessary.
- Documents all nursing treatment accurately and within departmental time requirements into participants’ charts.
- Understands and demonstrates respect for participant rights and utilizes established mechanisms for management of ethical issues in participant care.
- Records participant care delivered as planned and any variation, with appropriate rationale. Makes and records observations, as well as implements nursing measures related to impending or associated problems.
- Participates as an active and cooperative member of the Interdisciplinary Team if needed. Attends staff meetings and takes part in participant care planning.
Qualifications
- Graduate of an accredited school of nursing.
- Current LPN license in good standing to practice in the required state.
- Minimum 3 years LPN clinical experience with either one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the Jordan Valley Senior Care on working with a frail or elderly population upon hiring.
- Relevant experience must be within the last 3 years.
- Minimum 1-year wound care experience.
- Other training as required by the department.
- Current BLS certification required within 90 days of hire.
