Essential Duties & Responsibilities
- Complete necessary documentation for nursing assessments and planning evaluations.
- Evaluate clients as necessary to ensure client needs align with the care plan.
- Utilize professional assessment skills to identify physical, mental, and psychosocial needs and develop a care plan.
- Coordinate with management and RNs to implement and revise operational procedures for quality client care.
- Mentor peers and subordinates to maintain professional nursing standards.
- Act on behalf of clients to uphold patient rights.
- Be aware of home safety issues and ensure all responsibilities are completed safely.
- Respond promptly to client complaints and document them for review.
- Lead caregiver trainings at Senior Helpers of Northern Queens office.
- Manage caregivers and provide feedback and training as needed.

Physical Demands
- Able to lift, move, and assist clients.

- Able to operate office equipment such as a computer, phone, etc.
- Able to work in a state of alertness and safely.
Qualifications
- High school diploma.
- Licensed Registered LPN.
- At least two years of experience in home health care.
- Meet NY state requirements for caregiving/RN services.
- Valid and current state driver’s license and insurance.
