Essential Duties & Responsibilities

  • Complete necessary documentation for nursing assessments and planning evaluations.
  • Evaluate clients as necessary to ensure client needs align with the care plan.
  • Utilize professional assessment skills to identify physical, mental, and psychosocial needs and develop a care plan.
  • Coordinate with management and RNs to implement and revise operational procedures for quality client care.
  • Mentor peers and subordinates to maintain professional nursing standards.
  • Act on behalf of clients to uphold patient rights.
  • Be aware of home safety issues and ensure all responsibilities are completed safely.
  • Respond promptly to client complaints and document them for review.
  • Lead caregiver trainings at Senior Helpers of Northern Queens office.
  • Manage caregivers and provide feedback and training as needed.

Physical Demands

  • Able to lift, move, and assist clients.
  • Able to communicate with others to accurately exchange information.
  • Able to operate office equipment such as a computer, phone, etc.
  • Able to work in a state of alertness and safely.

Qualifications

  • High school diploma.
  • Licensed Registered LPN.
  • At least two years of experience in home health care.
  • Meet NY state requirements for caregiving/RN services.
  • Valid and current state driver’s license and insurance.
