Responsibilities
- Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
- Aids in preparing reports as required.
- Follow-up on 100% of all positive results for cancer within 72 hours and DETEC cases must, within two months respectively.
- Provides supportive services such as eligibility determination, enrollment assistance, and patient education.
- Reports data to supervisor daily and meets minimum productivity standard.
- Tracks and manages patients to ensure timely care needs are completed and follow-up care is received as indicated and documented.
- Performs other duties as assigned.
Requirements
- Bachelor's degree in a health-related field required or 4 years of experience in healthcare.
- Experience/Specialized skills (including Language): Basic computer literacy, ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.), basic organizational skills, attention to detail, time-management skills, and motivation to meet deadlines and achieve goals.
- Bilingual in English/Spanish preferred.
- Excellent interpersonal and customer service skills.
- Excellent written and verbal communication skills.
- Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.
- Ability and means to travel as needed in a timely manner within San Diego County.
Driver Requirements
- Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended.
- An acceptable driving record is also required.
- California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency.
- Mileage and other reimbursement governed by policy.
