Primary Function
Responsible for providing patient care as appropriate in the clinical setting, for medical case management, coordination of services with other agencies, clinical tracking of special population groups and any other activities or duties required to assure optimal patient care services.
Physician Principal Duties and Responsibilities
- Provides compassionate and comprehensive patient care services to a diverse population in a patient and family centered environment.
- Adheres to professional standards, practice policies, and procedures, federal, state and local requirements and regulatory standards.
- Complies with regulations regarding consultations and other services as required by patients' insurance coverage.
- Complies with Joint Commission standards for ambulatory care.
- Provides consultations by examining patients; reviewing medical records, reports; researching information and developing conclusions and care recommendations.
- Orders, performs, and interprets tests and analyzes records, reports, and examination information to diagnose patients' condition.
- Works as a team player, provides clinical leadership, supervision and act as a resource to nurse practitioners and other members of the health care team and community.
- Works with center manager to facilitate smooth operation of clinic flow.
- Provides appropriate documentation of clinical care provided in a timely manner. This includes maintenance of patient charts in accordance with clinic protocols, and timely documentation of clinical procedures on encounter forms.
- Participates in quality assurance initiatives and provides input on the development of policies and procedures which impact the health delivery service.
- Affirms quality of patient care by participating in the organization’s Performance Improvement activities.
- Cooperates with compliance review activities.
- SUBMIT INFORMATION NEEDED FOR CREDENTIALING FILES IN A TIMELY MANNER.
- Maintains confidentiality and complies with Health Insurance Portability and Accountability Act (HIPAA).
- When needed, assists the Safety Officer with employee health activities, especially in the event of an exposure to bloodborne pathogens.
- REFERS PATIENTS TO MEDICAL SPECIALIST OR OTHER PRACTITIONERS WHEN NECESSARY.
- ATTENDS ALL REQUIRED PROVIDER MEETINGS, PARTICIPATES IN QUALITY ASSURANCE AND OTHER CLINICAL MEETINGS AND TRAINING PROGRAMS.
- PERFORMS ANY AND ALL OTHER DUTIES REQUIRED TO PROVIDE FOR OPTIMAL PATIENT CARE.
Nurse Practitioner Principal Duties and Responsibilities
- Provides direct patient care as needed and appropriate to serve the community in accordance with the organization's Health Care Plan.
- Provides clinical leadership and acts as a resource to other members of the health care team, patients, or other members of the community.
- Works with center manager to facilitate smooth operation of clinic flow.
- Provides appropriate documentation of clinical care provided in a timely manner. This includes maintenance of patient charts in accordance with clinic protocols, and timely documentation of clinical procedures on encounter forms.
- Affirms quality of patient care by participating in the organization’s Performance Improvement activities.

- WHEN NEEDED, ASSISTS THE SAFETY OFFICER WITH EMPLOYEE HEALTH ACTIVITIES, ESPECIALLY IN THE EVENT OF AN EXPOSURE TO BLOODBORNE PATHOGENS.
- ATTENDS REQUIRED AGENCY MEETINGS.
- PARTICIPATES IN TEAM/STAFF MEETINGS; ATTENDS INSERVICE MEETINGS AND APPLICABLE WORKSHOPS.
- PERFORMS ALL OTHER DUTIES REQUIRED TO PROVIDE FOR OPTIMAL PATIENT CARE.
