ESSENTIAL FUNCTIONS
- Case Management and assessment of consumers assigned by Case Management Supervisor. This includes CCMEP.
- Maintains complete records on all consumers receiving services as assigned.
- Performs intake for consumers.
- Mets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database.
- Attends meetings and case staffings as required for each consumer.
- Coordinates daily consumer schedules.
- Prepares required reports on all consumers and trainees.
- Provides progress reports for the Case Management Supervisor on a weekly basis for everyone served.
- Works directly with consumers and trainees in all phases of the rehabilitation process.
- Provides retention services to consumers.
- May function as a driver when needed.
- Performs other duties as assigned by the Case Management Supervisor and the Mission Services Director.

- Complies with all established work rules, policies, procedures and safety rules.
- Represents the organization in a professional and courteous manner.
- Is able to educate stakeholders and community about the entire agency.
QUALIFICATIONS
- Education: College degree in a Human Services-related field preferred, but not required (High school diploma or equivalent with relevant experience accepted).
- Experience: 3 years in a related field preferred.
- Certification: CPR/First Aid (can be obtained upon hire).
- Other: Valid driver’s license and insurability or ability to provide reliable transportation.
