WFS SSVF Rural Case Manager

About the role

This position works with referred veterans and their families to assess housing resources, needs, and barriers and make an appropriate housing plan cooperatively with the veteran. The role involves providing case management services to veterans to develop and execute Individual Housing Plans (IHP) that enhance housing security for eligible participants through direct and indirect service provision in Goodwill (GIIN) and partner agencies.

Responsibilities

  • Screen Veterans for eligibility into the Support Services for Veterans' Families (SSVF) program.
  • Take referrals from community providers and community residents.
  • Establish and document participant eligibility for the program. Determine the participant’s household needs and priorities. Gather and record information to identify and coordinate those services that will be most effective and acceptable to the participant in improving housing stability. Provide alternate resources and diversion assistance to those veterans not eligible.
  • Ensure that participants receive the assistance needed to promote housing stability until they exit the program and that participants obtain needed supportive services.
  • Cookordinate activities and work collaboratively with contracted community agencies providing services under the SSVF program to ensure that IHPs are achieved.
  • Use informal community networks to locate resources and make referrals as necessary in support of IHPs.
  • Conduct other services and activities in concert with Department of Veterans Affairs, SSVF Families program guide and at the direction of Goodwill Program Manager.
  • Aid in the development and implementation of programs and services.
  • Aid in the screening of referrals for appropriate program and service options.
  • Follow up on participant progress, providing encouragement.
  • Ensure all paperwork and related activities are current, accurate, and in compliance.
  • Enter required data into Participant database in a timely and accurate manner.
  • Stay informed of state and federal laws affecting the organization and the people it serves.
  • Aid participants in achieving the highest level of independent functioning while receiving services.
  • Facilitate Person-Centered Planning.
  • Inform business, labor, and the general public about Goodwill services and available potential employees.
  • Develop knowledge of local housing resources, housing eligibility criteria, and housing types.
  • Locate landlords willing to lease to clients with housing barriers.
  • Need to network with other community agencies to determine rental and deposit assistance that might be beneficial to program participants.
  • Inform Program Manager of any unusual events that occur, and document appropriately for incidents.
  • Make necessary notifications when required to law enforcement, supervisors, and other appropriate individuals.
  • Advocate for the veteran with community resources, provide community referrals, and mediation (housing/crisis intervention).
  • Maintain flexibility to work evenings or weekends shifts.
  • Demonstrate knowledge of community and the community resources.
  • Work collaboratively with WFS to provide participant services.
  • Follow all policies, procedures, and directives of GIIN assuring safety of personnel or property.
  • Properly wear and maintain all required Personal Protective Equipment (PPE).
  • Maintain agency confidentiality. Violation of confidentiality is cause for immediate dismissal.
  • Comply with all health, safety, and fire standards and all local, state, and federal regulations (WISHA & OSHA).

Requirements

Must pass background and driver record check.

Qualifications

  • Bachelor's degree from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
