About the Role

The Wellness Outreach Clinician serves on the Outreach Health Coach team, assisting with health assessments, education, and consultations for Outreach customers. This role involves facilitating health fairs, wellness programs, classes, and education events, as well as conducting interviews, participating in screenings, and reviewing health or wellness plans with patients.

Responsibilities

  • Performs assessments including psychosocial, sensory perception, function/mobilization, skin integrity, age-related changes, and nutritional needs.
  • Obtains patient history and completes patient safety checklists (e.g., fall risk) within the scope of licensure.
  • Develops relationships with participants by providing support, encouragement, and education to help them work toward better health.
  • Documents identified teaching needs and implements plans, including patient and/or significant other education.
  • Discusses appropriate screenings (e.g., mammogram, colorectal, cervical, dental) with participants as needed.
  • Reviews lifestyle profiles, gathering information on medications, medical history, social habits, and health concerns.
  • Uses knowledge and skills to discuss wellness topics such as health-related behaviors, medications, medical history, family history, and health promotion resources.
  • Collects specimens and performs simple finger pricks if necessary.
  • Obtains vital signs, height, and weight of participants.
  • Operates an AED if needed.

Requirements

  • High School diploma and 1-2 years of formal training/education.
  • Bachelor’s or Master’s degree in allied health or a related field strongly preferred.
  • American Heart Association BLS certification required.

Pay

Pay range: $24.69 - $35.99 per hour. Starting pay in the Outer Banks Hospital/Outer Banks Medical Group market may be higher to reflect local market competitiveness.

Schedule

Shift expectations: Monday through Friday, with occasional weekends and nights.
