In this role, you’ll support the delivery of exceptional resident care, provide clinical guidance to the care team, and help ensure residents receive personalized, attentive service every day. Responsibilities

  • Team Leadership and Staffing
    • Ensure daily Resident Assistant assignment sheets are completed and followed
    • Provide hands-on training and orientation for newly hired CMAs, medication technicians, nurses, and resident care team members
    • Supervise team members to ensure care is delivered according to company policies and each resident’s individualized service plan
    • Reassign team members as needed to address call-outs, scheduling changes, or changes in resident census
    • Support nurses and medication technicians with clinical questions and resident care needs
    • Provide meal and break coverage for the day-shift nurse as needed
  • Clinical Oversight
    • Participate in daily clinical stand-up meetings
    • Assess residents experiencing changes in condition and ensure appropriate documentation and communication with families and physicians
    • Review the daily 24-hour communication log and follow up on resident changes and required charting
    • Review the HOT Box and ensure all necessary clinical follow-up is completed
    • Conduct daily rounds and provide clinical guidance throughout the community
    • Support the Evergreen Director with clinical oversight and resident care needs
    • Organize relevant clinical information for visiting physicians and nurse practitioners
  • Medication and Documentation Management
    • Review and approve medication orders in QuickMAR
    • Ensure monthly resident weights and vital signs are completed and entered accurately
    • Complete quarterly self-administration assessments for residents who manage their own medications
    • Review new resident records for accuracy and completeness using the required checklist
    • Complete monthly medication cycle check-ins for Assisted Living, Bridges, and Evergreen
    • Review medication discrepancy reports and ensure concerns are resolved promptly

Requirements

  • Current Georgia LPN/LVN or RN license in good standing
  • Previous clinical experience in assisted living, memory care, senior living, or a related healthcare setting preferred
  • Strong leadership, communication, assessment, and documentation skills
  • Ability to effectively supervise, train, and support nursing and resident care team members
  • Familiarity with electronic medication administration systems, including QuickMAR, is preferred
