Wellness Nurse (LPN)
About the role
Village Crossing at Worman's Mill offers a shift every Monday from 7:00 AM - 7:00 PM. The position is available at Solera Senior Living, a company guided by Core Values of Compassion, Commitment, Communication, and Creativity.
Responsibilities
- Provide direct nursing care and supervise medication care specialists.
- Administer medications, treatments, and procedures per physician’s orders.
- Document resident care and update care plans as needed.
- Monitor for changes in residents’ health and communicate updates to physicians and families.
- Aid with resident move-ins and orientation.
- Lead and guide care staff, including performance evaluations and coaching.
- Participate in emergency response procedures and uphold safety standards.
- Maintain accurate resident records and ensure compliance with community policies and state regulations.
- Promote resident independence, wellness, and participation in daily activities.
- Foster a positive, respectful environment for residents, families, and team members.
Qualifications
- Active LPN license in the state of employment.
- Minimum 1 year of nursing experience; experience with Alzheimer’s/dementia preferred.
- Strong communication, interpersonal, and leadership skills.
- Caregiver-focused approach to care.
- Strong observational skills and attention to detail.
- Annual flu vaccination required (medical/religious exemptions accepted).
