Position Summary
The Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our residents. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team.
Responsibilities
- Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
- Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
- When necessary, participate in hands-on care, medication administration, and specific nursing tasks.
- Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
- Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
- Assist in the transcription of physicians' orders and the review of new physicians' orders.
- Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
- Caregiver planning with home health agencies on-site, working within community and company policies.
- Address residents’ special needs, requests, and complaints.
- Coordinate admissions with the sales team, including supervising move-ins.
- Understand the community's care regulations along with state-specific regulations.
- Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
Requirements
- This LPN Position will be a 12hr Shift Candidate
- Qualifications:
- An incumbent nursing license (LPN/LVN) is required.
- A High School Diploma or equivalent is required, or a combination of education and experience.
- An associate degree or higher in nursing is preferred, or a combination of education and experience.
- Additional coursework in management is an asset.
- A current First Aid and CPR license or ability to obtain is required.
- At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
- At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.
- Experience with memory care is a plus.
