Visit Nurse - Home Care (Per Diem)

Provide nursing services to Hudson Valley Hospice patients and their families, ensuring care aligns with the agency’s philosophy, goals, objectives, and policies. Work closely with the interdisciplinary team, particularly RN Coordinators, to meet patient needs and visit requirements.
Responsibilities
- Maintain accurate records of all patient and family care services as defined by policy.
- Assess the needs of terminally ill patients and their families; participate in developing and implementing the Interdisciplinary Team Plan of Care.
- Collaborate with interdisciplinary team members to address patient needs.
- Revise the Plan of Care as needed, including medications, projected visits, Advanced Directive status, functional limitations, and treatments, in collaboration with the RN Case Manager or Coordinator.
- Provide 24-hour continuous skilled nursing care for patients and families in their homes as required.
- Assist Case Managers, Coordinators, and DPS in managing and supporting patient caseloads, responding to patient and family needs according to the Plan of Care.
- Maintain working knowledge of the CADD-Prizm pump.
Requirements
- Ability to communicate effectively, both verbally (in person or by phone) and in writing.
- Capability to work effectively and cooperatively, both independently and as part of a team.
- Expertise in prioritizing and handling multiple tasks efficiently.
- Experience caring for terminally ill patients and a commitment to the hospice philosophy.
- Working understanding of medications, treatments, and therapies for pain and symptom control.
- Bilingual skills are a plus.
Work Environment
- Moderate degree of possible exposure to highly contaminated agents.
- Regular travel by car in all weather conditions.
- Work in patient homes, hospitals, and nursing homes across counties as needed.
- Moderate to high risk of exposure to unsafe environments.
- Routine duties involve potential direct exposure to blood or body fluids.
- Ability to carry up to twenty-five pounds of equipment and/or supplies into a patient’s home and assist in transferring or positioning an average adult for care.
Qualifications
- Graduate from an accredited nursing program.
- Valid NYS Nursing License.
- Minimum of two (2) years of nursing experience.
- Experience in oncology, home care, or a hospice setting preferred.
- Experience working in a comprehensive interdisciplinary team approach to care preferred.
Schedule
- Per Diem: 6 shifts per month (2 shifts must be weekend days).
- 1 holiday per year.
- 8:30 AM – 4:30 PM (flexible hours available if needed).
Pay
$39–43 per hour, depending on experience.
