About the role

As our Virtually Integrated Care RN, you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You will bridge care gaps and enhance engagement in a dynamic virtual environment by assessing patient needs, developing personalized care plans, and providing direct clinical coaching. You will proactively monitor progress, coordinate diverse services, and leverage telehealth while tracking compliance, documenting interactions, and contributing to quality improvement.

You will collaborate virtually with physicians, nurses, and other healthcare team members to provide clinical patient care, including virtual assessment of medical, physical, and psychological status, and respond to routine, urgent, and emergent patient issues. You will oversee patient care plans to implement preventive measures, improve patient safety, and enhance the quality of care delivered via telemedicine technology. Additionally, you will drive risk reduction through participation in root cause analysis (RCA’s), quality surveillance, and data interpretation.

Responsibilities

  • Collaborate with unit leadership and oversee unit quality programs and metrics.
  • Ensure evidence-based practices are followed by team members to provide high-quality care and safety for patients.
  • Support management to ensure clinical operations comply with internal policies, standards, procedures, and regulatory requirements.
  • Continuously assess services, patient outcomes, quality control, and patient/family satisfaction to ensure optimal care delivery.
  • Identify the need for changes in standards, programs, and services, and develop recommendations for leadership.
  • Monitor processes in collaboration with bedside staff and provide recommendations for care delivery improvement.
  • Participate in developing strategies, initiatives, and programs addressing standardization, patient outcomes, team engagement, and nurse satisfaction.
  • Provide oversight of physiological trends and telemedicine technology to enhance patient care.

Requirements

  • Bachelor of Nursing (BSN) or actively pursuing a BSN degree upon hire.
  • Minimum of 3 years of overall acute care nursing experience.
  • Minimum of 2 years of nursing preceptor experience.
  • 1-3 years in a nursing leadership role (Charge RN role applicable).
  • Registered Nurse (RN) licensure in Arizona.
  • Basic Life Support (BLS) certification.

Pay

$35.00 - $58.63 per hour.
