Job Summary

Cabell Huntington Hospital is seeking a casual, as needed Virtual Integrated Care Coordinator (RN) for their CHH Virtual Integrated Care Department. The role supports and drives the plan of care, providing comprehensive remote oversight of patient care.

Responsibilities

• Supports and drives the plan of care
• Provides comprehensive remote oversight of the care of patients on a unit
• Collaborates with the direct care team and virtual care members to anticipate and direct the care of the patient through evaluating health promotion, risk reduction, and advanced clinical decision-making
• Affirms accountability for patient care outcomes through the assimilation and application of evidence-based practice to design, implement, and evaluate care
• Coordinates, delegates, and supervises the care provided by the unit-based direct care team
• Demonstrates professional competence and autonomy in performing the roles of clinician, educator, consultant/collaborator, and leader
• Performs related duties as required
