Virtual Integrated Care Coordinator (RN)

About the role

This job is responsible for supporting and driving the plan of care and providing comprehensive remote oversight of the care of patients on a unit. The virtual model includes collaboration between the direct care team and virtual care members where the Virtual RN will anticipate and direct the care of the patient through evaluating health promotion, risk reduction and advanced clinical decision-making.

Responsibilities

• Assumes accountability for patient care outcomes through the assimilation and application of evidence-based practice to design, implement and evaluate care.
• Coincides with the direct care team to coordinate, delegate and supervise the care provided.
• Demonstrates professional competence and autonomy in performing the roles of clinician, educator, consultant/collaborator, and leader.
• Performs related duties as required.

Requirements

• Must be able to complete duties at the bedside if needed.

Qualifications

• Current licensure as a Registered Nurse in West Virginia.
• Experience in a hospital setting preferred.
• Experience with virtual care coordination preferred.
• Strong communication and problem-solving skills.

Skills

• Proficiency in electronic medical records systems.

Schedule
