About the role
The Viral Hepatitis & TB Care Coordinator supports NEMS' Viral Hepatitis B/C and Tuberculosis (TB) Programs through patient care coordination and outreach.
Responsibilities
- Conducts targeted telephone and/or face-to-face outreach and education for hard-to-reach and high-risk communities.
- Provides education on viral hepatitis B/C, TB, other communicable disease prevention, testing, vaccination, and support services.
- Schedules primary care, radiology, vaccine, and other appointments for patients.
- Plays a key role in identifying and understanding the needs, interests, and barriers affecting current and prospective patient populations to increase awareness and utilization of services offered by NEMS.
- Motivates and advocates for patients to be active and engaged in their health.
- Practices cultural humility and adopts a trauma-informed approach to increase confidence in vaccinations and treatment options.
- Conducts data collection, tracking, and reporting of outreach activities and outcomes related to grant programs, research, and other initiatives.
- Collaborates with local/regional health departments, academic and community partners, and other stakeholders to ensure program goals are achieved.
Requirements
- High school diploma with minimum of 2 years of relevant work experience. Associate or Bachelor’s degree preferred.
- Prior experience with medical appointment scheduling and with Epic electronic health record strongly preferred.
- Prior experience with customer service; can build rapport with patients via phone.
- Previous experience with viral hepatitis B- or tuberculosis-related work is a plus.
- Proficient with Microsoft Office, Excel, PowerPoint, and Outlook.
- Ability to follow through with delegated tasks and self-manage in a detail-oriented environment.
- Good organization and prioritization skills to manage multiple projects and timelines.
- Strong interpersonal skills and communication to collaborate effectively with a broad range of internal and external stakeholders.
- Excellent time management skills.
Qualifications
- Fluency in English.
- Fluency in Chinese (Cantonese or Mandarin).
- Fluency in reading and writing Chinese preferred.
- Fluency in other languages is an asset.
