About the role
The Health Care for Homeless Veterans (HCHV) Outreach Social Work Coordinator is based in the SFVA Health Care System (SFVAHCS) catchment area. This position focuses on identifying and engaging Veterans who are experiencing homelessness or marginally housed and underutilizing VA services.
Responsibilities
- Provide a range of social work services, including assessment and referrals, clinical treatment, short-term case management, advocacy, service coordination, and linkage with VA and community resources.
- Engage in outreach activities to increase resources for "on-the-street" or marginally housed Veterans, aiming to improve their stability and help them return to independent and healthy lifestyles.
- Facilitate VA involvement in the Coordinated Entry System (CES) and assist SFVAHCS in meeting federal CES guidelines.
- Conduct bio/psycho/social assessments of Veterans, highlighting their strengths, limitations, and service needs.
- Understand the diverse treatment and needs of Veterans experiencing homelessness, including OIF/OEF, geriatric, and gender-specific Veterans.
- Document assessments using appropriate ROIs and complete a HOMES assessment, revising assessments as necessary to meet goals in the initial intake plan.
- Create an Individualized Service Plan (ISP) outlining Veteran strengths, short and long-term goals, measurable objectives, referral plans, and historical details of their needs.
- Offer case management services tailored to individual needs, ensuring Veterans' requirements are met.
- Outreach to homeless Veterans and work with city/county agencies to increase resources for them, aiming to improve their stability and lifestyle.
- Advocate for Veterans when it serves their best interest, educating and encouraging them to advocate for themselves.
- Provide education about VA and community resources, advance directives, and refer Veterans to appropriate interdisciplinary team members for health education needs.
- Offer ongoing supportive counseling to assist Veterans in coping with homelessness and understanding barriers to housing.
Requirements
- Eligible for the Education Debt Reduction Program (EDRP).
- Must be proficient in written and spoken English.
- Must pass a pre-employment physical evaluation.
- Must be willing to undergo a background/security investigation.
- Must participate in the seasonal influenza vaccination program.
- Must have selective service registration if applicable.
Qualifications
- Master's degree in social work from a CSWE-accredited school.
- Licensed or certified as an Advanced Practice Social Worker.
- Knowledge of program coordination and administration, including consultation, negotiation, and monitoring.
- Ability to write policies, procedures, and practice guidelines for the program.
- Ability to supervise multidisciplinary staff and provide training, orientation, and guidance within clinical practice.
