VA- Pharmacist

Core Responsibilities

• Carry out comprehensive medication reviews to ensure safe and appropriate medication therapy.
• Evaluate medication regimens to identify potential drug interactions, contraindications, or duplications.
• Collaborate with providers and clinical care teams to optimize medication therapy plans.
• Provide clinical recommendations supporting evidence-based medication use.
• Maintain medication adherence and identify opportunities to improve compliance.
• Support medication therapy management (MTM) programs.
• Ensure medications align with formulary requirements and clinical guidelines.
• Offer education to members regarding medication use, side effects, and treatment expectations.
• Collaborate with interdisciplinary teams to support coordinated patient care.
• Implement population health initiatives through medication management strategies.
• Assist in identifying cost-effective medication alternatives when appropriate.
• Support prior authorization review processes as applicable.
• Ensure medication practices align with regulatory and organizational standards.

Qualifications and Education

• Doctor of Pharmacy (PharmD) or Bachelor’s Degree in Pharmacy required.
• Active Pharmacist license in the Commonwealth of Virginia required.
• Minimum of 2 years of clinical pharmacy experience preferred.
• Experience supporting medication therapy management programs preferred.
• Knowledge of Medicare, Medicaid, and healthcare regulatory requirements preferred.
• Strong knowledge of pharmacology and medication safety practices.
• Experience working with electronic medical records (EMR) systems preferred.
• Strong analytical and problem-solving skills.
• Strong communication and collaboration skills.
