Core Responsibilities
- Carry out comprehensive medication reviews to ensure safe and appropriate medication therapy.
- Evaluate medication regimens to identify potential drug interactions, contraindications, or duplications.
- Collaborate with providers and clinical care teams to optimize medication therapy plans.
- Provide clinical recommendations supporting evidence-based medication use.
- Maintain medication adherence and identify opportunities to improve compliance.
- Support medication therapy management (MTM) programs.
- Ensure medications align with formulary requirements and clinical guidelines.
- Offer education to members regarding medication use, side effects, and treatment expectations.
- Collaborate with interdisciplinary teams to support coordinated patient care.
- Implement population health initiatives through medication management strategies.
- Assist in identifying cost-effective medication alternatives when appropriate.
- Support prior authorization review processes as applicable.
- Ensure medication practices align with regulatory and organizational standards.
Qualifications and Education
- Doctor of Pharmacy (PharmD) or Bachelor’s Degree in Pharmacy required.
- Active Pharmacist license in the Commonwealth of Virginia required.
- Minimum of 2 years of clinical pharmacy experience preferred.
- Experience supporting medication therapy management programs preferred.
- Knowledge of Medicare, Medicaid, and healthcare regulatory requirements preferred.
- Strong knowledge of pharmacology and medication safety practices.
- Experience working with electronic medical records (EMR) systems preferred.
- Strong analytical and problem-solving skills.
- Strong communication and collaboration skills.
