Job Summary
Reviews prescriptions to ensure accuracy, assess needed ingredients, and evaluate suitability. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage. Counsels patients on medication administration, possible side effects, expected response, and storage. Monitors patient health and progress related to drug therapy.
Responsibilities
- Interprets and dispenses both prescription and non-prescription medication to patients in a retail/community pharmacy setting.
- Takes an active role in addressing prescription issues and advising providers on medication selection, dosage, interactions, and side effects.
- Counsels patients on medication prescription, including proper administration, side effects, expected response, and storage.
- Maintains oversight and clinical supervision for pharmacy series and support personnel.
- Utilizes appropriate age-related patient information to assess pharmaceutical needs, including neonatal, pediatric, adolescent, adult, and geriatric patients.
- Fulfills responsibilities in various departments within a medical center or institutional setting, including acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation.
- Engages in decentralized and centralized clinical functions, such as direct patient interaction, interdisciplinary team rounding, chart reviews, customer/patient counseling, collecting accurate medication lists, and overseeing IV room clinical functions.
Qualifications
- In a retail/community pharmacy setting: Graduated from an accredited School of Pharmacy; thorough knowledge of drugs and their actions; knowledge of state and local regulations.
- In a medical center or institutional setting: Graduated from an accredited School of Pharmacy; thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.
