About the role
This position provides direct case management services to the needy and disenfranchised in Ulster County through advocacy, information, referral, crisis intervention, and short-term case management. The role also includes reception duties such as answering telephones and processing clients for services.
Responsibilities
- Provide crisis intervention to homeless individuals and families facing housing emergencies.
- Serve as a primary case manager for the New York State Rental Supplement Program Plan (RSP), Shelter Arrears Eviction Forestallment program (SAEF), and emergency relief program.
- Conduct thorough needs assessments and develop service plans using a short-term intervention model.
- Perform social care screenings of Medicaid members using the standardized Accountable Health Communities (AHC) Health-Related Social Needs Screening tool on the Unite Us platform and refer cases as needed.
- Navigate individuals ineligible for enhanced services to non-Social Care Network services (local, state, and federal programs).
- Log all client interactions in Unite Us, Apricot, and/or other required systems.
- Maintain accurate and complete work files, records, and documentation.
- Process cases within a 45-day timeframe.
- Answer telephones and provide information as needed.
- Conduct community outreach and perform data collection activities.
- Assist with food and kids’ pantry distributions.
- Participate in weekend and after-hours outreach activities, including community tabling events, pop-up pantries, and special projects (e.g., Stuff-The-Bus, Adopt-A-Family, Giving Tree, Back to School supply distributions).
- Develop and maintain working relationships with providers throughout Ulster County.
- Attend community or service provider meetings.
- Participate in regular supervisory sessions and agency in-service training.
- Travel to meet clients at other CCOSU offices as needed.
- Perform other duties as assigned by the supervisor or program demands.
Requirements
- Bachelor’s degree in Social Work or related field (preferred) or Associate’s degree with one year of experience in Human Services.
- Clean NYS Driver’s License, maintained throughout employment.
- Knowledge of social services available in Ulster County (preferred).
- Computer literacy with proficiency in Microsoft Word, Excel, and Outlook.
- Excellent interpersonal, communication, organizational, and time-management skills.
- Ability to work independently and as part of a team.
- Ability to work with the public and under potentially stressful conditions, including basic understanding of client engagement and de-escalation techniques.
- Ability to prepare comprehensive progress notes and service plans.
- Capable of remaining stationary at a workstation and using a computer for at least 30% of the time.
Pay
$23.00 per hour.
Schedule
This is a full-time position. Days and hours of work are generally 9:00 AM to 5:00 PM, Monday through Friday. Additional hours may be required to meet program deadlines or client needs. Availability for planned weekend and after-hours activities is expected.
