Case Manager - Macomb County

Job Summary

This position requires working in the community and performing home visits.

Requirements

• Bachelor’s degree in a human services field from an accredited institution required.
• LLBSW, LBSW, LLMSW, LMSW, LMFT preferred.

Responsibilities

• Cook up all services necessary to maximize consumer recovery and independence for individuals with mental illness.
• In a residential or clinical setting, provide clinical and program consultation to direct care staff.
• Ensure case management compliance with agency standards and the practice model.
• Aid consumer to improve quality of life and maintain the highest possible ability to recover and function within the community.
• Implement strategies for obtaining services and supports that are goal-oriented and individualized.

Qualifications

• Bachelor’s degree in a human services field from an accredited institution required.
• LLBSW, LBSW, LLMSW, LMSW, LMFT preferred.
• Prior case management experience in a mental health or rehabilitation setting preferred.
• Valid Michigan Driver’s License.
