Responsibilities
- Develop and deliver a full complement of services, including individual, family, group counseling, individual rehabilitation and case management/brokerage for individuals within the Care Coordinator's caseload as assigned.
- Utilize Evidence Based Practice for the designated population/program to assure high quality results and intended impact.
- Assess individual’s risk status including risk based on symptoms, substance use, environment, physical conditions, etc and intervene appropriately.
- Create a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
- Outreach and engage clients, report disengagement to the team immediately, and work with outreach team to determine when to disenroll individuals based on program protocols.
- Assess and coordinate domain-based assessments, referrals, service planning, community planning, and benefit coordination which supports wellness and recovery principles.
- Document and maintain all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.
- Participate in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by manager.
- Able to work independently and as a member of a therapeutic team.
- Adaptable and open to change and new information; adjusts behavior and work methods in response to new information, changing conditions, or unexpected obstacles.
- Balances interests of a variety of clients; readily readjusts priorities to respond to pressing and changing client demands; committed to continuous improvement of services.
Qualifications
- High School Diploma required and must have 4 years of paid work experience in mental health/social services. One of those years may be in a customer service function. Degree is preferred.
- Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage are required. DMV printout required.
- Extensive local travel is required, vehicle reimbursement is provided. Care Coordinators transport clients in their personal vehicle.
Compensation & Benefits
- Starting at $27.00 per hour, then $30.00 per hour after satisfactory performance and achievement of all job related goals and requirements for the 90 day introductory period.
- +3% per hour differential for specified language fluency. In Alameda County - Chinese, Cantonese, Spanish, Vietnamese, Mandarin, Farsi, Cambodian and Tagalog
