Pinnacle Services is seeking friendly, self-starting professionals to join our Ramsey County Case Management team. Responsibilities
- Assess areas of the person served where assistance is needed.
- Offer and coordinate resources for individuals supported.
- Monitor service delivery and collaborate with providers to ensure needs are met.
- Develop service plans.
- Attend regular meetings with persons served and their interdisciplinary teams.
- Document client-related work to create billable units.
- Work with persons served on a mix of CADI and DD waivers, who may have developmental disabilities, serious and persistent mental illness, chemical dependency issues, physical disabilities, or are aging.
Requirements
- Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field, or
- Graduate from an accredited four-year college in any field and one year of full-time experience as a social worker, case manager, or care coordinator in a public or private social service agency, or
- One year of experience in education or treatment of persons with developmental disabilities or related conditions, or
- At least one course focused on developmental disabilities (official transcript required for verification).
- Valid driver’s license, acceptable driving record, and a vehicle with current insurance.
- Successfully clear local and/or state background checks.
Qualifications
Preferred: Possess an LSW or higher form of BOSW Licensure.
Pay
Non-LSW wage: $49,000/year (salaried for 40 hours/week). LSW wage: $51,600/year (salaried for 40 hours/week).
