About the Role

This is a full-time, hybrid position requiring travel to community-based meetings multiple times per week and remote work from a home office. The role involves in-person meetings, so applicants must live within the State of Indiana or within 10 miles of the state line.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plans of care and collaborate with service providers as needed

Who You’ll Work With

Case managers work with Individuals on two Medicaid Waivers: Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH).

Requirements

• Valid driver’s license, car insurance, and reliable transportation
• Bachelor’s Degree in Human Services
• Minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
• Designated home office setting with computer, internet service, and smartphone
