Pinnacle Services is seeking a friendly, self-starting professional for its Washington County Case Management team. The position offers a dynamic, collaborative work environment and opportunities for personal and professional growth.
Job Responsibilities and Duties
- Assess the areas of the person served life where assistance is needed.
- Offer and coordinate resources for the person served being supported.
- Monitor service delivery and work with providers to ensure the person served needs are met.
- Develop service plans.
- Attend regular meetings with the person served and their interdisciplinary teams.
- Create billable units for client-related work.

Requirements
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field, or
- Be a graduate from an accredited four-year college with a major in any field and have one year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency, or
- Have one year of experience in education or treatment of persons served with developmental disabilities or related conditions, or
- Have at least one course focused on developmental disabilities is required.
Candidates must provide an official college or university transcript to verify completion of this coursework. Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance. Applicants must successfully clear local and/or state background checks.
Preferred Qualifications
- Possess a LSW or higher form of BOSW licensure.

Salary
$49,000 - $51,600 per year
Benefits
- Non-LSW wage: $49,000/year salaried for 40hrs/week.
- LSW wage: $51,600/year salaried for 40hrs/week.
