About the role

This .80 fte position will be based at our Hugo clinic with potential for coverage at our Roseville or Lino Lakes clinics. This position will work in clinic Monday, Tuesday, Thursday, Friday days (start time between 7:45am – 8:30am and end time between 4:15pm – 5:30pm).

Responsibilities

  • Triage support for our clinic at the North Suburban Family Physicians clinics by using protocols embedded in the electronic medical record and a variety of resources including standing orders and protocols.
  • Occasionally working with patients that have questions or needs once on-site.
  • Provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition.
  • Delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity.
  • Creates and implements dynamic teaching plans based upon identified patient need.
  • Acts as a resource for other staff.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current RN licensure in Minnesota and Wisconsin (within two months of hire).
  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained.
  • Strongly demonstrated written and oral communication skills.

Preferred Qualifications

  • One-year full-time employment as an RN.
  • Triage or Care Management experience within the last three years.

  • Nursing Process: Demonstrates clinical competence in using the nursing process to provide care to patients and their families.
  • Consultation/Collaboration: Shares information with other members of the health care team, makes appropriate patient referrals, uses interdisciplinary resources to enhance own clinical decision making, provides continuity among home, office, inpatient setting and community, uses appropriate systems, automated or manual, to obtain patient information or schedule appointments.
  • Care Coordination: Assists patients/families to optimally manage their health care, respects their culture and values, helps to identify individual needs, health goals and treatment preferences.
  • Education: Patient / Family / Community works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals, teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation, engages in ongoing evaluation of the effectiveness of teaching in terms of understanding and application.
  • Professional: Participates in patient care conferences, in-services and educational programs, keeps current on best practices related to nursing and patient care.
  • Leadership: Nursing leadership is the possession of skills and knowledge to manage situations, assess problems and facilitate solutions. It includes listening, decision-making, delegating, communication, motivating, facilitating, role modeling, critical thinking and problem solving.
  • Organizational Behaviors: Shows interest in and keeps informed of department and organizational changes within the North Suburban Family Physicians system. Provides honest, respectful and constructive input and feedback. Identifies and works to resolve problems with other departments and individuals in a constructive manner. Focuses on solving the problem, not blaming. Effectively shares job knowledge with new employees. Places team goals above own interests.
