Triage RN
This .80 FTE position is based at our Lino Lakes clinic with potential for coverage at our Roseville or Hugo clinics. The schedule is Monday, Tuesday, Wednesday, and Friday during daytime hours (start time between 7:45 AM – 8:30 AM and end time between 4:15 PM – 5:00 PM).
About the role
This position provides triage support for our clinic using protocols embedded in the electronic medical record, standing orders, and other resources. Responsibilities include:
- Assessing and addressing patient questions or needs upon arrival
- Conducting comprehensive assessments to establish and evaluate care plans
- Addressing changes in a patient’s condition
- Delegating nursing tasks when appropriate and supervising function or activity
- Creating and implementing dynamic teaching plans based on patient needs
- Serving as a resource for other staff
Responsibilities
Nursing Process
- Assessment: Accomplished through interviewing, observation, and/or nursing procedures to distinguish between normal and abnormal findings
- Problem Identification: Defines the impact of health problems on individuals and families
- Planning: Sets mutually acceptable goals with the nurse and all pertinent parties
- Implementation: Provides comprehensive nursing care following established policies, procedures, and standing orders
- Evaluation: Assesses and reassesses patient progress toward goal achievement

- Demonstrates clinical competence using the nursing process
- Implements nursing policies, procedures, and standing orders

- Documents accurately and completely
Consultation/Collaboration
- Shares information with the healthcare team

- Uses automated or manual systems to obtain patient information or schedule appointments
Care Coordination
- Uses health coaching techniques to develop individualized patient action plans
- Uses population management tools (e.g., registries) to identify high-risk patients and track their care
- Engages and educates patients and families to implement strategies for reaching health goals
- Identifies and secures patient services needed for care
- Informs and supports patients in making informed decisions and navigating the healthcare system
Education
- Patient/Family/Community: Partners with patients and families to identify needs, develop education plans, and make referrals
- Teaches techniques for prevention, self-care, or rehabilitation
- Evaluates the effectiveness of teaching
- Professional: Participates in patient care conferences, in-services, and educational programs to stay current on best practices

Requirements
- Graduate of an accredited school of nursing
- Current RN licensure in Minnesota
- Current RN licensure in Wisconsin (within two months of hire)
- Current BLS certification for health professionals through the American Heart Association or Red Cross (in-person certification required; online certification is not acceptable)
- Strong written and oral communication skills
Qualifications
Preferred:
- One year of full-time employment as an RN
- Triage or care management experience within the last three years
