About the role

The Ambulatory Triage LPN and Triage MA/LPN are responsible for addressing patient's condition/symptoms virtually, giving appropriate clinical direction, education, and recommendation(s) for disposition. When not in triage, the Medical Assistant will assist the provider with patient visits, procedures, laboratory tests, referrals, and quality outcomes.

Responsibilities

  • Address patient's condition/symptoms virtually, providing clinical direction, education, and recommendations for disposition.
  • Assist the provider with patient visits and procedures, routine laboratory tests, and referrals.
  • Manage exam rooms and treatment areas, including cleaning and stocking supplies.
  • Obtain vital signs, EKGs, and specimen collections.
  • Document care using the Nextech EMR system.
  • Perform Point of Care Testing based on patient's chief complaint and available protocols.
  • Process referrals and medication refills.
  • Maintain positive relationships with patients, providers, and staff.

Qualifications

  • One to two years of ambulatory and/or triage experience in a clinical setting (Preferred).
  • Possesses knowledge of necessary computer systems including Microsoft Office, Outlook, and network electronic medical records (EMR).
  • Supports the mission, vision, and PRIIDE values of Community Health Network.
  • Utilizes critical thinking skills, education, mentoring, guidance, and interventions to achieve goals of care virtually.
  • Serve as a scribe for the provider.
  • Prepare exam rooms and treatment areas for patient use, including required cleaning and stocking of supplies.
  • Room patients and manage an efficient flow of provider’s caseload.
  • Obtain necessary vital signs, EKGs, and/or specimen collections.
  • Maintain positive relationships with patients, providers, and staff.
