Treatment Foster Care Case Manager

About the role

Clarvida is seeking a Treatment Foster Care Case Manager to provide support and guidance to youth in foster care. The ideal candidate will have experience working with adolescents and a passion for supporting young people through challenging times.

Responsibilities

• Assess and document the needs of each foster child
• Develop and implement individualized treatment plans
• Maintain regular contact with foster parents and other professionals involved in the youth's care
• Coordinate services and resources to ensure the best possible outcomes for the youth
• Provide crisis intervention and support as needed

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field
• At least 2 years of relevant experience in foster care or social work
• Licensure as a Social Worker in the state of [State]
• Experience working with adolescents and trauma-informed care

Qualifications

• Strong communication and interpersonal skills
