About the Role
We are seeking a licensed Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based position emphasizes preventive care, addressing social determinants of health, and recognizing urgent situations requiring intervention.
Responsibilities
- Perform medical history reviews, physical exams, and screenings
- Document diagnoses and communicate findings to primary care providers (PCPs)
- Address social factors affecting health and provide referrals
- Recognize and respond to urgent or emergent health issues
- Educate patients on disease management and medication adherence
Requirements
- Active, unrestricted license in the practicing state
- Valid driver’s license and reliable transportation
- Ability to navigate home environments and perform assessments
- For NPs: Certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- For PAs: NCCPA certification and state licensure
Preferred Skills & Experience
- 1+ years of clinical experience, preferably in family, geriatric, or home health care
- Proficiency with electronic medical records and technology
- Strong communication skills with Medicare populations
- Ability to transport equipment and navigate stairs
Pay
Salary ranges from $91,700 to $163,700 annually, based on experience and location.
