Traveling Nurse or Physician Assistant

About the Role

We are seeking a licensed Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based position emphasizes preventive care, addressing social determinants of health, and recognizing urgent situations requiring intervention.

Responsibilities

• Perform medical history reviews, physical exams, and screenings
• Document diagnoses and communicate findings to primary care providers (PCPs)
• Address social factors affecting health and provide referrals
• Recognize and respond to urgent or emergent health issues
• Educate patients on disease management and medication adherence

Requirements

• Active, unrestricted license in the practicing state
• Valid driver’s license and reliable transportation
• Ability to navigate home environments and perform assessments
• For NPs: Certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: NCCPA certification and state licensure

Preferred Skills & Experience

• 1+ years of clinical experience, preferably in family, geriatric, or home health care
• Proficiency with electronic medical records and technology
• Strong communication skills with Medicare populations
• Ability to transport equipment and navigate stairs

Pay

Salary ranges from $91,700 to $163,700 annually, based on experience and location.

Approximately 85% travel across the state, with all expenses covered.
