Travel Social Work - Social Worker - $2,641 per week

Med Ninjas is seeking a Social Work Social Worker for a travel job in Morristown, New Jersey. Start date: ASAP. Duration: 13 weeks. 40 hours per week. Shift: 8 hours, days. Employment type: Travel. Provides therapeutic intervention and social work services to patients and families to support comprehensive, integrated, and uninterrupted care during hospitalization and transition to community-based care.

Responsibilities

• Conduct psychosocial assessments to identify patient and family needs related to medical conditions and social determinants of health.
• Interview patients, family members, and significant others to assess needs and develop appropriate interventions and treatment goals.
• Participate in multidisciplinary rounds and provide psychosocial input regarding patient care and discharge planning.
• Provide psychosocial support to patients and families during adjustment to illness, hospitalization, and life changes.
• Serve as a patient and family resource and advocate, including support with Advanced Care Planning.
• Collaborate with the healthcare team regarding inpatient needs, post-hospital services, and continuity of care.
• Coordinate comprehensive discharge planning, including referrals to LTC/Custodial Care, Acute Rehab, Sub-Acute Rehab/SNF, LTAC, Dialysis, Hospice, and homelessness resources.
• Connect patients and families with community and governmental resources, including financial assistance, food benefits, transitional housing, and other support services.
• Maintain accurate documentation of psychosocial assessments, patient contacts, family interventions, and discharge planning in accordance with hospital, state, federal, and Joint Commission requirements.
• Maintain confidentiality and professional standards while participating in program planning, quality improvement, and development of community social service networks.

Requirements

• Must have an active/valid New Jersey Social Work license at the time of submission.
• Hospital experience required.
• Inpatient adult discharge planning experience required.

Qualifications

• Master of Social Work (MSW) from an accredited school of Social Work.
