Travel Social Work - Social Worker - $2,451 per week

About the Role

Travel Social Worker position in Portsmouth, New Hampshire at Portsmouth Regional Hospital. This is a 13-week assignment starting 08/31/2026 with 40 hours per week, 8-hour day shifts.

Unit Information

• Variable units as needed within the main campus
• 25 beds, 1 staff member per shift
• Day shift: 8 AM–4:30 PM (night shift N/A)
• Patient ratio: 25:1
• EMR: Meditech Expanse

Responsibilities

• Hospital case management
• Hospital discharge assessment and planning (Home Health, SNF, IRF, wound vac, O₂, etc.)
• Float as needed within the hospital
• Work every other weekend or every third weekend (no call)

Requirements

• BSW or MSW
• Hospital Case Management experience (required)
• Hospital discharge assessment and planning experience (required)
• Acute care hospital case management experience (most recruited for)
• Excellent communication skills and resilience in a fast-paced environment

Preferred Qualifications

• Meditech Expanse experience
• Careport experience
