Travel Social Work - Master of Social Work - $2,779 per week

Responsibilities

• Complete comprehensive biopsychosocial assessments for patients across various age groups and care settings.
• Provide crisis intervention, short-term counseling, advocacy, and emotional support to patients and families.
• Develop and coordinate discharge plans in collaboration with the healthcare team.
• Aid patients and families with access to community resources and support services.
• Provide psychoeducation and facilitate support groups as needed.
• Serve as a key member of the interdisciplinary care team.
• Collaborate with physicians, nurses, case management, and other healthcare professionals to support patient-centered care.
• Provide consultation regarding psychosocial aspects of patient care.
• Educate hospital staff and participate in committees or quality improvement initiatives as requested.

Requirements

• Master of Social Work (MSW) degree required.
• Minimum 2 years of Medical Social Work experience required.
• Charge experience required.
• Previous inpatient acute hospital experience required.
• Strong experience with discharge planning and transitions of care.
• Excellent communication, assessment, and problem-solving skills.
• Ability to work effectively in a fast-paced healthcare environment.
