Responsibilities

• Provide social work services to patients and families in a hospital setting.
• Assess and address the psychosocial needs of patients and their families.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Conduct home visits as needed to provide continuity of care.

Requirements

• Masters of Social Work degree from an accredited program.
• Licensure as a Social Worker in the state of New York.
• At least 2 years of relevant work experience in a hospital or related healthcare setting.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Qualifications

• Current CPR certification.
• Knowledge of HIPAA regulations.
• Experience with electronic medical records (EMR).
