Responsibilities

• Provide social work services in accordance with professional standards and regulations.
• Assess client needs and develop care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Document patient information using OASIS and Epic Remote Client systems.
• Ensure compliance with all relevant healthcare policies and procedures.

Requirements

• 2 years of recent Skilled Home Health Social Work experience required.
• Active Oregon or Washington licensure required.
• LCSW required.
• BLS certification required.
• OASIS experience required.
• Experience with Epic Remote Client, Rover, and OASIS documentation preferred.
