Travel Social Work - LMSW - Licensed Master Social Worker - $2,430 per week

Travel assignment in Mission Viejo, CA (92691) – 13 weeks, 40 hours per week, 8-hour day shifts (8:00 AM – 4:30 PM).

Requirements

• Master’s Degree in Social Work (MSW)
• Active California MSW license (pending license not accepted)
• Minimum 2 years of acute-care hospital case management / discharge planning experience
• 2+ years of hospital experience
• Epic EMR experience
• Experience with HMOs, IPAs, Medicare, and Medicaid
• Travel experience preferred; first-time travelers may be considered

Responsibilities

• Provide medical social work services for hospitalized patients in an acute-care setting
• Complete initial psychosocial assessments within 24 hours of admission
• Develop and coordinate individualized discharge plans
• Coordinate post-acute care services (home care, rehabilitation, long-term care, durable medical equipment)
• Collaborate with physicians, nurses, utilization management, insurance case managers, and multidisciplinary teams
• Provide crisis intervention, patient and family education, advocacy, and community resource referrals
• Assist with advance directives, protective services referrals, and discharge planning for diverse patient populations
• Maintain timely and accurate documentation in Epic
• Float as needed
