Schedule

• Day Shift
• Monday – Friday
• 8:30 AM – 5:00 PM

Requirements

• Recent hospital-based Med/Surg experience required
• EPIC experience required
• Active LMSW or LCSW-C license required, or an equivalent active out-of-state social work license
• Candidates with an equivalent out-of-state license must have a completed application on file with the applicable licensing board for clearance to work

Responsibilities

• Conduct early identification and high-risk screening of patients
• Perform readmission risk screenings
• Provide counseling and support for patients adjusting to illness
• Complete safety assessments
• Coordinate and facilitate discharge planning
• Identify and address barriers to safe and timely discharge
• Collaborate with interdisciplinary healthcare teams to support patient care goals

Ideal Candidate

The ideal candidate has strong clinical assessment skills, experience working with complex inpatient populations, and the ability to coordinate care across multiple disciplines. Excellent communication, organization, and advocacy skills are essential.
