About the role
The Social Worker will provide comprehensive social work and care coordination services to patients from newborn to geriatric populations. The role involves assessing patient needs, developing care plans, coordinating resources, supporting safe discharge, and collaborating with an interdisciplinary healthcare team to improve continuity of care and patient outcomes.
Qualifications
- Master’s degree in Social Work (MSW).
- LMSW or LCSW license — current, valid.
- BLS certification from the American Heart Association.
- Minimum 3 years of experience in a clinical care setting.
- Experience with discharge planning, case management, or utilization review.
- Knowledge of electronic health record (EHR) systems.
- Experience working with multidisciplinary healthcare teams.
