LanceSoft is seeking a Social Work Licensed Clinical Social Worker for a travel job in Walnut Creek, California.
Responsibilities
- Provide clinical social work services in an outpatient setting
- Assess client needs and develop treatment plans
- Collaborate with interdisciplinary teams to provide comprehensive care
- Document patient interactions and progress in the charting system (Epic)
Requirements
- LCSW certification required
- Pediatric experience preferred
- Outpatient clinic and case management experience preferred
- Ability to float to other UCSF facilities
- Second language proficiency, Spanish preferred
Qualifications
- Experience working in an outpatient clinic setting
- Experience with case management
- Ability to work in an interdisciplinary team environment
Skills
