Wellmore of Daniel Island is seeking a Travel Physical Therapist Assistant (PTA) for a Skilled Nursing Facility assignment in Charleston, South Carolina. Located on sought-after Daniel Island, this opportunity offers a supportive rehab environment, consistent hours, and the chance to work with an adult and geriatric patient population focused on improving mobility, function, and independence.

Working under the direction of a Physical Therapist, you'll help implement treatment plans, assist patients with therapeutic exercises and functional mobility training, and play an important role in helping residents achieve their rehabilitation goals.

Responsibilities

• Carry out treatment plans established by the supervising Physical Therapist
• Assist patients with gait training, balance activities, transfers, and therapeutic exercise
• Monitor patient progress and communicate updates to the therapy team
• Educate patients and caregivers on home exercise programs and safety techniques
• Maintain timely and accurate documentation
• Collaborate with nursing staff and interdisciplinary team members

Requirements

• South Carolina PTA License Required
• BLS Required
• SNF experience preferred
