Travel Physical Therapist in a Skilled Nursing Facility setting.
Responsibilities
- Assist patients in recovery from injuries and management of chronic conditions
- Perform mobility training
- Fall prevention
- Maintain accurate documentation in electronic medical records
- Collaborate with interdisciplinary teams to provide effective rehabilitation care
Qualifications & Skills
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program
- Active Connecticut PT license
- Preferred experience in SNF or long-term care
- Proficiency in patient mobility, fall prevention, and documentation
- BLS certification may be required
