Travel assignment for a Registered Nurse in home health care, based in South Portland, Maine.
About the role
Home-based skilled nursing care with care coordination across interdisciplinary teams and referring agencies. This is a 14-week travel assignment starting on 09/21/2026.
Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings in the clinical record and communicate with the team
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate these into the POC
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are accurately stated and updated as changes occur
- Provide care with infection control measures and educate patients and families on care, medications, home safety, and self-care practices
- Supervise and provide clinical directions to HHA/LVN to ensure quality and continuity of service
- Participate in on-call responsibilities and interdisciplinary team meetings as assigned
- Communicate effectively as liaison to patients, families, and healthcare professionals
Requirements
- Current state RN license (Required)
- CHPN certification (Preferred); CPR certification (Required)
- 1–2 years of RN clinical care experience; 2+ years in home health (Required/strongly preferred)
- ADN or Nursing Diploma program graduate (Required); BSN preferred
- Homecare Homebase software experience (Required)
- OASIS experience (Required)
- Current driver’s license; reliable transportation and auto insurance; ability to travel locally (Required)
- Ability to assess, plan, implement, and coordinate care across diverse settings; strong communication and documentation skills
Pay
Weekly pay: $2,405–$2,568 per week. Guaranteed hours: 40 per week.
Schedule
14-week contract. Shift: 8 hours per day.
