Travel Registered Nurse – Home Health assignment in Saint Johnsbury, Vermont. 12-week contract starting 08/03/2026.
About the role
Practice in a home health setting that emphasizes community health education and patient-centered care, balancing independence in the field with a collaborative care team.
Responsibilities
- Conduct initial and ongoing nursing assessments in the patient’s home; determine home care needs and develop a comprehensive plan of care with goals in collaboration with patient and family.
- Administer medications and treatments as prescribed; provide direct patient care within the scope of practice and ensure adherence to the State Nurse Practice Act.
- Re-evaluate nursing needs regularly, adjust the plan of care as patient status changes, and initiate discharge planning as appropriate.
- Document care through clinical notes; communicate with the physician and care team to report changes and obtain orders as required; coordinate with community health resources to support the plan of care.
- Serve as Case Manager when assigned, coordinating patient care for an assigned caseload and arranging equipment and services as needed.
- Provide patient and family education on health maintenance, disease management, and home safety; support continuity of care across settings.
Requirements
- Active registered nurse license to practice in Vermont; CPR certification.
- Knowledge of the Oasis data set.
- One (1) to two (2) years of acute care experience preferred.
- Graduate of an accredited nursing program.
- Valid driver’s license with reliable, insured vehicle; ability to travel to patient homes in the community.
- Ability to perform essential home health duties: prolonged walking/standing, lifting or transferring patients, and lifting up to 50 pounds; ability to bend, reach, and kneel as needed.
- Willingness to work under stress in emergency situations and during irregular hours; exposure to communicable diseases and bodily fluids.
Pay
Weekly pay: $2,459 - $2,594.
Schedule
- Guaranteed hours: 40 hours per week.
- Shift: Monday through Friday.
