Responsibilities
- Deliver professional nursing care to home-based patients using the nursing process; assess patient and caregiver needs and develop an evidence-based plan of care
- Serve as Case Manager for assigned patients, actively managing caseloads and visit times in line with agency standards
- Complete and maintain accurate OASIS documentation in accordance with agency policies and regulatory requirements
- Implement the medical plan of care and nursing interventions; maintain infection control standards and timely documentation
- Collaborate with LPNs and other disciplines to ensure the plan of care is followed and patient needs are met; participate in IDT meetings as required
- Educate patients and caregivers on medications, treatment options, discharge instructions, and safety considerations; coordinate safe discharges or transfers
Requirements
- Current, active RN license in Vermont (Vermont State Board of Nursing) or equivalent with eligibility to practice in Vermont
- OASIS experience required
- Prior home health and hospice experience preferred; strong assessment, care planning, and documentation skills
- Proficient in high-tech nursing skills (IV use and maintenance, wound care including wound VAC, complex wound care) and infection control practices
- Able to work effectively in a home-based setting; strong communication and teamwork skills
- Some on-call and weekend coverage required
