Responsibilities
- Provide comprehensive nursing care through the nursing process, including initial assessments and ongoing reevaluation of physical, psychosocial, environmental, and community resources
- Initiate and modify Plan of Care in collaboration with the patient, physician, other disciplines, and caregivers; establish discharge plans
- Supervise CNA and coordinate care with other health care workers as assigned
- Complete OASIS data and deliver care in alignment with federal, state, and agency regulations; maintain patient privacy and confidentiality
- Assess educational needs of patients, families, and caregivers; implement appropriate teaching strategies; document per regulatory guidelines
- Maintain a clean, safe care environment and uphold high standards of privacy and confidentiality; adhere to Standards of Conduct
Requirements
- Active Illinois registered nursing license (must be active at submission; copy to be included in submission)
- Home Health experience required; OASIS and start-of-care experience strongly preferred
- BLS certification (active or willing to obtain)
- Valid driver's license and car insurance
- Covid-19 and influenza vaccines required; vaccination status must be documented in profile (declinations not approved)
