Travel Registered Nurse Home Health Job
Overview
This travel assignment starts on 08/03/2026 for 12 weeks, with a weekly pay range of $2,447–$2,542 and guaranteed 36 hours per week. Shift: Days. Setting: home health and hospice services serving Caledonia and southern Essex counties.
Responsibilities
- Provide a full range of professional nursing services to clients of all ages in the home, including admission, evaluation, and care plan development
- Collaborate with a multidisciplinary team of home care professionals to coordinate care and ensure high-quality, safe outcomes
- Conduct comprehensive nursing assessments, administer medications, monitor vitals, manage wounds, and address chronic conditions per the individualized plan
- Document assessments, interventions, and patient responses accurately; communicate effectively with patients, families, and the care team
- Educate patients and families on disease management, medications, treatment plans, and safety measures; advocate for patients and support families throughout care
- Ensure compliance with regulatory standards and agency policies; participate in quality improvement and ongoing professional development activities
Requirements
- Active Vermont RN license (current, in good standing)
- Minimum 1 year of prior clinical nursing experience
- Ability to provide home health/hospice nursing care and participate in home visits
- Effective assessment, communication, and documentation skills
- Ability to work the listed shift and travel within the service area as required
